For part 1 of the project, you must come up with a scenario that centers around a fictitious patient that you will be treating. You must use the course resources to indicate your patients:  initials, social history and health history (include the disease/ disorder). Part 1 Project Guidelines:

For this part of your project you will write a report to discuss the following information:

• Patient Initials – Do not provide your patient’s name because you will want to maintain patient confidentiality, but use his or her initials.
• Patient Social History – Write at least one paragraph to discuss your patient’s social history in terms of the following:
  • Family life
  • Occupation
  • Recreational aspects of their personal life
• Health History – Describe the health history of the patient in at least 200 words. Include the following information:
  • Diagnosis – Discuss the diagnosis of your patient in detail.
  • Patient’s chief complaint
  • History of present illness
  • Allergies
  • Medications they are taking
  • Past medical history
  • Past surgical history
  • Psychiatric history
  • Family history of illness
