In terms of literature, clinical reasoning, decision making and critical thinking are in most cases used interchangeably. The utilization of pain assessment tools for the adults who fluently are able to speak and communicate English is easy. The patients are able to give their consent concerning the treatment they got. However, the elderly adults who have cognitive impairment will just be confused and hence are not able to produce a reliable measurement of the intensity of pain (Barrows , Norman , Neufeld , & Stephanie, 2015).
