ED Boarding
Inpatient boarding is when patients have to wait in the emergency department (ED), waiting for a room assignment in the inpatient ward. Inpatient boarding causes crowding in the ED, increased wait times, patients leaving without being seen from the waiting room, poorer patient outcomes, and higher mortality rates (Carmen, Nieuwenhuyse, & Van Houdt, 2018, p. 954). Inpatient boarding occurs due to lack of staff, equipment, and beds or because the bed in the inpatient ward is not yet ready (Shi, Cou, Dai, Ding, & Sim, 2015, p. 1). Nurse leaders need to continually be evaluating new ways to combat boarding issues in the ED and increasing throughput in all areas of the hospital. One possible solution is to implement a Discharge Hospitality Center where discharged patients who met specific criteria would be placed at this center waiting for transportation or social resources (Smith, 2018, p. 283). Transporting patients up for discharge to the hospitality center would allow acutely ill patients waiting in the ED to take their place in the inpatient room.

I first analyzed what the biggest safety concern at my facility is currently. Our biggest issue at this time is long wait times caused by ED boarding. I used Walden University’s library database to research what boarding is, the adverse effects of boarding, and the causes of boarding. In the search bar, I typed: boarding and nursing and emergency department or causes or effects or solution. I refined my results to by specifying full text, peer-reviewed scholarly journals only, in the past five years. I was able to find all three articles using this search criterion. Using Walden’s library database, it is simple to research peer-reviewed journals and use them in scholarly writing. Using peer-reviewed articles ensures the validity of the research and further supports the information being presented.

ED Boarding

The posting defines ED boarding, its causes and effects. It notes that it is the transient continued use of ED facilities when the inpatient facilities are not available. It also mentions that it is caused by deficiencies in the inpatient department that could range from personnel shortage to v lack of equipment and beds. It further adds that this has serious implications for the ED that is forced to work beyond its mandate and capacity with the result that patients who should be in the ED end up being negatively affected. Polit and Beck (2017) express similar sentiments but adds that ED boarding is simply a unique management solution that should only be used occasionally. In fact, it mentions that this strategy should not be used since it creates a lot of problems that diminish its value. McGonigle and Mastrian (2018) offer similar sentiments by noting that ED boarding is only used when the inpatient department cannot handle new patients and they must be temporarily housed in the ED even as space is created for them in the inpatient department. However, there is a need to acknowledge that ED boarding is an indication of management inability to manage patients’ transition between the different departments. As a business entity, each medical facility has intimate knowledge of its capacity in terms of how many patients can be housed in each department. With this knowledge it is possible to plan the use of the ED and inpatient department so that no overcrowding is experienced. Hynes et al. (2010) supports this idea by mentioning that health information technology can be leveraged to understand patient movement within the facility and facilitate planning efforts. In this respect, ED boarding is resultant of planning shortcomings and an inability to leverage health information technology to determine facility capacity.
