The field healthcare engages a wide range of healthcare professions that require skilled and knowledgeable personnel for quality delivery of healthcare services. Nursing profession is among these professions that play a crucial role in delivery of health services directly to patients. Due this essential role, the institute of medicine (IOM) merged with the Robert Wood Johnson Foundation (RWJF) in the year 2008 with an aiming of completing a 2-year project that focused on rectifying and improving the nursing practice in the United States (Hassmiller & Reinhard, 2019). The IOM played a role in instituting the RWJF for the purpose of report writing. The situation of nursing practice during the time of report writing made the IOM to have an urge of developing the appropriate recommendations that would transform nursing. The main goal of the report was to therefore develop the recommendations that would positively transform the practices of nursing in the entire US. The various barriers that exist and prevent effective nursing practices from positively impacting the various health problems made the IOM to develop the RWJF committee. After the RWJF committee was enacted it played an essential role of exploring in all the healthcare facilities such as nursing homes, city hospitals, clinics and public health centers. The committees action of exploring these healthcare settings made them realize that different nurses from different settings had different levels of competencies, knowledge and skills. Four key messages contained in the IOM report
Nurses are the some of the most utilized health professions in the field of healthcare. Nurses spend most of the 24hours providing services to their patients compared to other healthcare providers such as physicians and practitioners. There was therefore the need to develop various recommendations that would help in advancing rectifying and nursing practice. The first key recommendation is that a mandatory full training and education should be a key observation while executing nursing practice. It is a recommendation that focuses on the redesigning of the nursing scope at state and national level. The practicum taught in nursing schools should be rectified to help the nurse to meet the emerging health issues in any healthcare setting. The recommendation therefore focuses on the skills and knowledge provided to nurses at various nursing schools in national and state levels. The second key recommendation is that nursing practice requires high levels of training and education to be provided to nurses. Higher levels of skills and knowledge facilitate the provision of safe and patient centered care by nurses (Box-Steffensmeier et al., 2018). The higher levels also enhance the release of committed and patient oriented nurses from nursing schools to different healthcare settings. The third key recommendation is that nurses should always work together with their practitioners towards improving the healthcare system. It is a recommendation that plays a role in promoting the leadership capabilities between nurses and their practitioners. It therefore helps to nurture the leadership potentials among the regular nurses in their respective settings. Finally, the last recommendation is that a good data handling and data infrastructure is a requirement in policy making and workforce planning. The main focus of the recommendation is increasing the number of highly qualified nurses such as baccalaureate and doctoral degree holders.
The campaign for action
The campaign for action was a program that was introduced at national and state level with an aim of emphasizing on the need to implement the IOM recommendations. The goal of the campaign in relation to the IOM report was to initiate the implementation of the recommendations. However, the campaign’s ultimate goal was to guarantee the US citizens of quality and patient centered care through the full extent participation of nurses in healthcare delivery services. The campaign runs in every individual state through the state based coalitions. It is a campaign that is composed of the citizens, states, and the national institutions. There are various organizations that are engaged in the campaign. Some of these include the American Nurses Association (ANA), RWJF, AARP foundation, the champion nursing in America (CCNA) and the initiative of the AARP. They all work towards a full implementation of the IOM report recommendations at state levels (Ebell et al., 2019). The members of the campaign for coalition include philanthropic institutions and communities, 48 action coalitions, policymakers, academics and businesses.
Rationale for state-based action coalition
The different individual states have different laws and regulations that governs how their individual healthcare system should run. The state-based coalitions were mainly implemented to ensure that more private organizations and individuals participate in the initiative. Through the CCNA and the RWJF guidelines, quality procedures for providing delivery of uniform healthcare programs are provided (Hassmiller & Reinhard, 2019). The CCNA also have different roles in different sates due to the fact that the health legislation and policies also differ from one state to the other. Both the CCNA and the RWJF provide communication tools in all the states. The communication assistance among different states is therefore supported by the two bodies. The Action Coalition of Georgia
One of the successful initiatives of the action coalition of Georgia was a program that was virtually done by the Georgia Nursing Leadership Coalition (GNLC). The aim of the program was to increase the number of enrollments of nurses for doctoral programs. The GNLC formed an online program with the Augusta University and the Georgia State University (Ebell et al., 2019). The program consisted of mixed class meetings and engaged discussions intensive research experiences and education of new ways. The program was successful as it enrollments in doctoral programs increased in the first two years. The diversity in Georgia’s doctoral programs also increased by five percent since the introduction of the program. The students who participated detailed that the program was advantageous as it allowed the exchange of ideas between the students and the two institutions. However, one disadvantage was that the program was virtual and would not allow a one on one interaction among the students which would be more appealing.
Inadequate skills and political influence of policy development is one of the major barrier that hinders the nurses’ participation in policy development. Most nurses in the State of Georgia are not well equipped with the knowledge of policy development because of the low number of APRNs. Political influence also acts as a barrier where most political leaders make health related promises to citizens but fail to deliver their promises. Some of the ways of overcoming these barriers include offering excellent education and training as well as electing committed and political leaders whose aim is bettering the health system of the Georgians.
