In 2008, the Robert Wood Johnson Foundation (RWJF) and the Institute of Medicine (IOM) established a committee with a two-year lifespan with the intention of exploring the Future of Nursing. The primary mission of the committee initiative was to produce a report with recommendations for an action oriented blueprint on the ideal future of nursing profession to include recommendations for changes in institutional and public policies at the local, state and national/federal levels. To produce the IOM report, the committee initiative examined and presented recommendations that identified the vital role of nurses in the design and implementation of more efficient and effective health care systems. The efforts by the committee initiative were framed by the question of what roles can nurses assume to address the increasing demand for effective, high quality and safe health care services. In addition to this question, the committee sought to expand the role of nurses. The committee recognized that nurses play a crucial role with respect to eth value, accessibility and quality of care. However, it noted that the nursing profession must be subjected to fundamental changes in order for nurses to fulfil that crucial role. The IOM report presented four key messages targeted at transforming the nursing profession. Firstly, nurses should be allowed to practice to the full extent of their education and training. This message has galvanized fundamental changes in the roles of nurses with some of the states allowing nurses to enjoy full practice authority, reduced practice authority, and restricted practice authority. This expectation has caused fundamental changes in nursing education as nurses must be prepared for full practice authority. Secondly, nurses should achieve higher levels of training and education through an improved education system that promotes seamless academic progression. The current education programs allow nurses to advance in their education even while working. This is especially seen in distancing learning programs that allow nurses to acquire advanced education qualifications using internet services. Thirdly, nurses should be full partners with other medical professionals who include physicians, in redesigning the US health care system. Nurses are increasingly being consulted in changes being made to the health care system, such as nurses being consulted in the acquisition of clinical decision support systems (CDSS) to ensure that nurses needs are met as end users. Finally, effective workforce planning and policy making that requires better data collection and improved information infrastructure. This has become more evident with nursing informatics that leverages data to support nursing care delivery (Institute of Medicine, 2011).

State-based action coalitions help to drive change locally by facilitating partnership between nurses and other stakeholders to transform health care through nursing and present healthier local communities. These coalitions carry out campaign activities that involve them advocating for nurses and the local community. They help to advance the goals specified in the IOM report by working towards presenting a country whose citizens live healthier lives even as nurses are supported to provide care and promote wellbeing and health equity. The specific issues concerning efforts to strengthen nursing that are addressed by the coalition to advance the goals specified in the IOM report include collecting workforce data, promoting nurse leadership, fostering inter-professional issues, transforming nursing education, increasing diversity in nursing, and improving access to care (Campaign for Action, 2020a).

Virginia Action Coalition is working towards implementing solutions for important issues that affect the nursing profession within the state. It focuses on building nurse-based solutions for improving health care quality while transforming how the state’s residents receive health care. The coalition engages in some initiatives towards this end. One such initiative is the Virginia Nurses Foundation 40 Under 40 Award Winners that awards outstanding young nurse leaders who have made significant achievements in professional leadership and practice with an impact on the patients and communities. This initiative has been instrumental in promoting nursing leadership within the state. The second initiative is the SYNC program that focuses on improving inter-professional leadership. In partnership with other organizations, the coalition has presented SYNC as a learning institute that teaches health care teams strategies for improving effective leadership, leading teams, and better serving the communities. This initiative has been instrumental in fostering inter-professional collaboration within the state (Campaign for Action, 2020b).

  1. Describe barriers to advancement that currently exist in your state and explain how nursing advocates in your state overcome these barriers.

There are significant barriers to advancement that currently exist in the state of Virginia. The first barrier is the limited opportunities to advance. Medical organizations are grappling with the financial reality of increasing costs and are exploring strategies for reducing costs. A key area that has been targeted for reducing costs is staffing whereby organizations are increasingly opting for cheaper staffing options although they may present some risks. This means that the more knowledgeable and skilled nurses may be overlooked as they demand higher wages/remunerations. This can be addressed by standardizing the qualifications for each nursing role and responsibility so that the more qualified and skilled nurses do not compete with their less qualified and skilled counterparts. The second barrier is poor work environment. There is an existing shortage of nurses with the employed nurses being overworked and having to work for longer hours. This has increased dissatisfaction levels among nurses as well as personnel turnovers as nurses turn to less demanding careers. This can be addressed by improving the staffing plans (Milstead & Short, 2019).
