A 35 year old male patient presented to the nurse practitioner’s clinic to establish care. Patient is a retired veteran and recently had a honorable discharge from the Military. Mental Exam Documentation He stated that since being back home full time, he has had problems with sleeping, irritability, and often shouts at his kids at the least provocation. The nurse practitioner noted during his history taking that he confirms waking up at night time in cold sweats after having night mares. He also told that the nurse practitioner that during his time in Iraq, he lost two of his battle buddies. He feels bad because they went on an assignment together and he survived. He denies any Suicidal or Homicidal Ideations during the visit.
