Gordon’s functional health pattern was proposed and developed by Marjory Gordon. It is the method used by nurse to provide a comprehensive assessment on the client. Gordon’s functional health pattern is divided into 11 categories. These categories are a systematic and standardized approach to data collection. Each of the categories enables the nurse to determine the different factors of health and human function. These categories are health perception and health management, nutrition and metabolism, urine and waste elimination, activity and exercise, cognition and perception, sleep and rest, self-perception and self-concept, roles and relationships, sexuality and reproduction, coping and stress tolerance, and values and belief (Functional.
The second is category is nutritional and metabolic. This refers to the client’s usual pattern of food and fluid intake, diet, oral cavity health, and weight loss or gain. The data collected is focused on the pattern of food and fluid consumption. This includes the types of food eaten, the amount, and the frequency. The adequacies or inadequacies of the nutrient supplies are also evaluated. This may help to determine the actual or potential problems related to fluid balance, tissue integrity, malnutrition, or other gastrointestinal problems. The risks in this category include blood sugar problems, impaired fluid levels, impaired organ functions and aspiration (Koshar, N.D.). A sub category for this pattern is risk for unstable glucose. The nurse would ask the client “How often do you check your blood glucose?” A nursing intervention for the category is for the client to monitor their blood sugar before and after meals. A second intervention would be for the client to lose twenty pounds within the next 3 months (Gordon, 2007). The third category is urine and waste elimination. This refers to the client’s bowel and bladder elimination. The data collected is focused on elimination patterns, control problems, the use of assistive devices and the use of medications. While performing self health assessment, I found some problems in my behavior, which is mainly caused by stress due to new environment, new place, new rules and regulation. Study and distance between me and my family are the other factors that lead to stress. In this focused assessment, I am focusing in assessing level of stress.

I browse internet to gain more knowledge on my stress level. I assessed my stress level using “life change index scale/ The Stress Test”, written by Thomas H. Holmes and Richard H. Rahel. This stress test has three different rows including event, impact score and my score. As event adds up, there occurs increase in score. The higher the score, the chance of becoming ill will also be higher and also the change of returning back to normal health will decrease.

Health is considered to be the precious wealth of an individual. It is an important aspect of our life.
