Nursing Informatics
The benefits of health technologies being integrated into clinical systems has long been established even as new technologies are being leveraged to improve patient safety and care efficiency while reducing costs to present a complete and more proactive care. However, there is a lack of practical knowledge on how to link the clinical effectiveness of the new health technologies to return on investment and return on investment that has slowed down the adoption of these technologies. For health care to evolve and move forwards, these new technologies must be successfully adopted with organizations sharing failures and successes, learning from the failures and successes collectively, avoiding repeating mistakes, and saving human lives, costs and time (Lau & Kuziemsky, 2016). The present paper offers an analysis of four current journal articles identified to inform the subject of how clinical systems have impacted the outcomes and efficiencies within the context of nursing practice and health care delivery.

This is an eight-page journal article that explores the opportunities and challenges most commonly perceived in clinical information systems. The authors note that clinical information systems have been particularly useful for improving workflow and patient care although these improvements are relative. However, they explain that the benefits that can be realized from these systems are being overshadowed by a lack of functionality that creates data exchange, management and integration challenges. Towards this end, the study applied a secondary research approach that evaluated publications from medical publication databases. The identified publications were reviewed, synthesized and summarized to describe the current stage of the clinical information systems with the intention of assessing the current state of knowledge while identifying the associated challenges and benefits. The results of the literature review determined that clinical information systems can support evidence-based practices, and this incidentally improve efficacy of care, quality and patient safety. It is for these reasons that the adaptability, acceptability and advancement of clinical information systems have increased. Although the demand for the functionality of the systems is increasing, there is are data integration challenges as well as lack of functionality to exchange medical information that hinder the systems. The authors acknowledge that health information technology has high potential for improving care within complex adaptive health systems. However, they introduce new challenges and consequences with the more significant challenge being safety in the clinical settings. The study identifies nine significant challenges. The first challenge is how to conduct an effective risk assessment since the nature of these technologies are unique and require new assessment tools, methods and models to be developed. The second challenge is that the many technologies present different user interfaces that different across the technologies. There is a need to develop standard user interface design functions and features. The third challenge is that the software that powers these technologies present technology concerns especially when considering their interface and network capabilities that present opportunities for hackers. The fourth challenge is how to unambiguously identify the patients while conforming with information privacy and confidentiality expectations. The fifth challenge is how to develop and implement a decision support system to improve safety. The sixth challenge is how to identify practices for safely managing transitions between information technology systems. The seventh challenge is how to develop real-time methods for enabling automated surveillance and monitoring of system safety and performance. The eighth challenge is how to establish a culture and legal framework for sharing information about adverse events and hazards. The final challenge is how to develop method and models for safely improving health information technology. The nine challenges are identified as important controls that must be in place to ensure that health information technology delivers on its promise of patient safety. This is a five-page journal article that explores the effects of information technology on health care and their effects on patient outcomes and care delivery. Towards this end, the authors applied a secondary research approach that reviewed publications from medical publication databases. Based on the literature review approach, the study identified 34 publications that explored the research topic. Firstly, the study identified poor user interfaces and use error as factors that interfered with information receipt and led to decision-making errors. Secondly, the authors determine that clinical errors from medications are a significant concern. Thirdly, the article identified system functionality as a concern with the use of health technologies, to include delayed care delivery, fragmented displays, and user interfaces. Finally, issues with software updates, system configuration and system access are noted to delay care delivery. The five reported problems are linked to patient harm and death, as well as near miss events. This article is useful for exploring the problems with health technology with regards to risks to patient safety. The study is largely qualitative having applied a systematic approach, and this may limit the value of its results with results to how current the results are. Still, the authors are academicians and this improves the value of the study as they can be trusted to present an objective study. Overall, the study is well researched and descriptive thus making it useful for informing the subject of how clinical systems have impacted the outcomes and efficiencies within the context of nursing practice and health care delivery.

This is a nine-page journal article that explores how clinical data systems have been applied to support health care practice. It acknowledges that these systems are increasingly being adopted by health care organizations as software and data storage systems. The systems are being leveraged to improve medical practice. To explore this topic, the researchers evaluated the systems applied by local health departments. Towards this end, it was determined that 38.4% of departments make use of electronic health records. The use of technology was linked to coverage areas for the organization with those that serve large populations more likely to make use of health information technologies. In addition, it notes that although medical organizations are adopting health information technologies, 28.9% of clinical data and 59.4% of non-clinical data are managed in paper records. The study makes it clear that medical organizations are increasingly making use of health information technologies to store clinical data. Significant challenges to the use of technologies focus on the technology characteristics that include existing informatics capabilities, service portfolio, governance, and size of the organization.
