The Clinical Inquiry Process Using the PICOT Model .

In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question. To Prepare: Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry. Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources. The Clinical Inquiry Process Using the PICOT Model .

Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least two different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles. Review the Resources for guidance and develop a PICO(T) question of interest to you for further study. It is suggested that an Intervention-type PICOT question be developed as these seem to work best for this course. Post a brief description of your clinical issue of interest. The Clinical Inquiry Process Using the PICOT Model .This clinical issue will remain the same for the entire course and will be the basis for the development of your PICOT question. Then, post your PICO(T) question, the search terms used, and the names of at least two databases used for your PICO(T) question. Describe your search results in terms of the number of articles returned on original research and how this changed as you added search terms using your Boolean operators. Finally, explain strategies you might make to increase the rigor and effectiveness of a database search on your PICO(T) question. Be specific and provide examples. The Clinical Inquiry Process Using the PICOT Model .

The Clinical Inquiry Process Using the PICOT Model

Evidence-based practice (EBP) is built through clinical inquiry. But clinical inquiry in itself is actualised using the PICOT model (Melnyk & Fineout-Overholt, 2019). In this paper, the clinical issue of interest is the high readmission rate for heart failure (HF) patients aged 65 years and above within 30 days of discharge. The Clinical Inquiry Process Using the PICOT Model .Based on this problem, the PICOT question will be: “In HF patients aged 65 years and above and discharged from hospital (P), can a multidisciplinary nurse-led educational team (I) compared to the usual care (C) reduce the rate of readmission (O) within a period of 30 days post discharge (T)?” the search terms used in the search for peer-reviewed studies were “heart failure,” “readmission,” “30 days,” and “nurse-led intervention.” Two of the research databases used in searching for scholarly evidence in this case were CINAHL and PubMed (Walden University Library, n.d.-a).The Clinical Inquiry Process Using the PICOT Model .

The key terms were entered into the search engines of the databases to find the studies that corresponded to them. In the first instance, a total of 33,526 studies showed up. This was before filtering the search by date of publication, exclusion of systematic reviews and meta-analyses, and using Boolean operators (Walden University Library, n.d.-b). After combining the search terms with the Boolean operator “AND” as well as using the other filters mentioned, the total number of studies retrieved narrowed down substantially to 24 articles. These were now very specific articles dealing with interventions that address the practice problem in very specific terms. The Clinical Inquiry Process Using the PICOT Model .

The strategies that one might apply to increase the rigor, specificity, and effectiveness of a database search on this PICOT question include (i) making use of the in-built filters in the database. An example of this is to filter the search to produce only randomized controlled trials or RCTs. (ii) Using synonyms of search terms in repeat searches. This ensures that even the articles written using synonymous terms but relevant to the identified problem are captured in the search. The Clinical Inquiry Process Using the PICOT Model .
