Discussion – Week 1
March 21, 2010, was not EBP’s date of birth, but it may be the date the approach “grew up” and left home to take on the world.
When the Affordable Care Act was passed, it came with a requirement of empirical evidence. Research on EBP increased significantly. Application of EBP spread to allied health professions, education, healthcare technology, and more. Health organizations began to adopt and promote EBP.
In this Discussion, you will consider this adoption. You will examine healthcare organization websites and analyze to what extent these organizations use EBP.
To Prepare:
Post a description of the healthcare organization website you reviewed. Describe where, if at all, EBP appears (e.g., the mission, vision, philosophy, and/or goals of the healthcare organization, or in other locations on the website). Then, explain whether this healthcare organization’s work is grounded in EBP and why or why not. Finally, explain whether the information you discovered on the healthcare organization’s website has changed your perception of the healthcare organization. Be specific and provide examples.
Respond to at least two of your colleagues on two different days by visiting the websites they shared and offering additional examples of EBP or alternative views/interpretations to those shared in your colleagues’ posts.

Evidence-based Practice
The American Heart Association’s mission is to help people live healthily and provide ways to improve the way we eat to prevent strokes and heart attacks. AHA does not mention evidence-based but instead uses the word science in its mission or vision for a better life for everyone. The organization provides us with the tools necessary to maintain a lifestyle based on scientific-based research. According to (American Heart Association [AHA], 2022), they rely on science to fuel their fight for a healthier heart and are dedicated to ensuring the best researchers are on their team. They focus on finding the most innovative scientific-based research to make sure we are provided with the best ways to care for our patients and ways to take care of our personal lives. (American Heart Association [AHA], 2022), states that “through the 10 Commitments, the AHA is investing $100 million in new research programs and grants focused on scientific-based solutions to health inequities.” According to (Melnyk & Fineout-Overholt, 2019), evidence-based practice helps to reduce healthcare costs, leads to a higher quality of care, and improve the best patient outcome.” For example, some of AHA goals and achievement include “the first implantable pacemakers, the first artificial heart valve, CPR techniques, and cholesterol-lowering medications” (American Heart Association [AHA], 2022). I could not imagine being uneducated on the techniques for CPR; I have used it several times throughout my career for my patients. It makes you wonder what would happen if they did not research the first artificial heart valve or pacemakers. What would we do? Evidence-based practice has changed the way we practice in our professions; the American Heart Association is a change agent that has provided us with ways to live longer and healthy through their research.
I associated them with only heart and stroke prevention when I thought about the AHA. This information has changed the way I view this organization because I did not know that they were the ones who provided us with the best CPR techniques and the first implantable pacemakers. I knew my CPR card said BLS provider American Heart Association, but I never really researched why. According (Earls, 2017), “the first fully implantable pacemaker was given to a patient in 1958 at the Karolinska Institute in Solna, Sweden,” based on research funded by the AHA. This organization has done a lot for healthcare and will continue to dedicate its research to changing lives.

Hi Albert!
I enjoyed reading your post; I agree with what you wrote; yes, the American heart association is more like a road map that defines goals and has the steps needed to follow to reach the milestone. The American heart association guides the nation on daily consumption to have a healthy heart. Having healthy habits leads to healthy living. According to the online article, the American heart association’s mission is ” to be a relentless force for a world of longer, healthier lives ( AHA mission, vision, and the 12 essential elements that guide the AHA research program)”. By creating a guideline for everyone to follow and forming healthy habits, the American heart association allows everyone to live a healthier and longer life. This organization practices evidence-based practice using facts, not fiction or guesswork. AHA is committed to making life healthier for the nation by investing money and programs to help patients with cardiovascular issues. ” We are working toward improving the cardiovascular health of all Americans by 20 percent, and reducing deaths from cardiovascular diseases and stroke by 20 percent, all by the year 2020 ( American Heart Association)”.
Your post was informative and educational. I had no idea that AHA originated the CPR techniques.
Great Post!

Hello Albert,
The American Heart Association does indeed list its missions in a meaningful way. They try their best to humbly explain their background and their contribution to their missions. It is said themselves, Heart disease is the No. 1 killer worldwide, and stroke ranks second globally. Even when those conditions don’t result in death, they cause disability and diminish the quality of life. We want to see a world free of cardiovascular diseases and stroke. (About Us, 2021) They ranged from topics for eating, donating, health topics, tips, and even CPR lessons. Maps are shown on where people can get lessons if they ever need to save a life. It is stated from them that The American Heart Association’s CPR & ECC inspires the world to save lives and envisions a world where no one dies from cardiac arrest. The AHA is the leader in resuscitation science, education, and training, and the publisher of the official Guidelines for CPR and ECC. (American Heart Association CPR & First Aid, 2022)

Albert,
I am pleased you have chosen American Heart Association (AHA) for your discussion post this week. It was very informative. Heart disease can cause more death amongst women than men. The AHA started a movement geared to improve women’s cardiovascular health called “Go Red for Women.” It was developed to make women more aware of heart health and all and all the initiative’s goal is to improve all lives of women around the world (GoRedforWomen, 2022). Did you know heart disease can be preventable? All we have to do is educate our patients, friends, and family about this health threat. On February 4, 2022, a campaign was started to get lawmakers involved in community change increasing awareness of the number one killer of women. Teenagers are encouraged to participate through fundraising opportunities. AHA also has a Journal of American Heart Association that published peer-reviewed articles. “AHA has grown into the nation’s oldest and largest voluntary organization dedicated to fighting heart disease and stroke” (American Heart Association, 2022).

Hi Albert,
I enjoyed reading your comprehensive post. Evidence –based practice is now widely recognized as key to improving healthcare quality and patient outcomes. It is a result of information obtained through systemic reviews, randomized control trials, experimental studies, expert opinions, and laboratory research (Stannard, 2019). Evidence-based practice has changed the way we practice in our profession. The American Heart Association (AHA)is a change that has provided us with ways to live longer and healthy through research.
I agree that AHA website provides relevant information based on evidence-based practice. AHA’s mission is to help people live healthily and provide ways to improve your eating habits to prevent stroke and heart attacks. This organization practices evidence-based practice using facts, not fiction or guesswork. AHA is committed to making life healthier for the nation by investing $100 million in new research programs and grants to help patients with cardiovascular disease. Hi Hope,
Thank you for sharing your knowledge about infertility. Infertility is a common condition that is associated with significant psychological burden. Many couples will seek to increase their fertility with lifestyle changes before consulting a specialist.
When couples cannot achieve pregnancy, they often seek health care from medical and nursing specialists. The care the couple receives begins with a thorough assessment to determine the possible cause of infertility and to plan appropriate care to ensure the best chance for the couple to have a biological child. In this article, we provide an overview of the etiology and evaluation of infertility, the various treatment options available, and the appropriate clinical implications.
I wanted to add that the treatment of unexplained infertility is by necessity empiric. For most couples, the best initial therapy is a course (typically 3 or 4 cycles) of ovarian stimulation with oral medications and intrauterine insemination (OS-IUI) followed by in vitro fertilization for those unsuccessful with OS-IUI treatments.

RE: Discussion – Week 1
Hello, Hope. I really enjoyed reading your discussion post. It really touched me. Infertility is such an important topic that is not talked about as often as it should be. Although, it can be difficult to talk about if one is experiencing this devastating diagnoses. According to the National Institute of Health, 9% of men and 11% of women within the reproductive age experience infertility (NIH, 2018). That is a large number of people unable to conceive children. The website that you discussed in your post, The American Society for Reproductive Medicine Practice Committee, is very informative and filled with evidence-based practice. I think it is a great resource for individuals and couples who are dealing with this. They can use this reliable information to help them get through such a difficult or find a remedy to their predicament. Another website that is similar to The American Society of Reproductive medicine is National Infertility Association. It is also known as RESOLVE. This organization exists to provide infertile couples with access to care, advocacy, support, education, and awareness (RESOLVE, 2022). I think it is important to have websites like these that are reliable and grounded on evidence-based practice. It really gives couples a feeling of hope and that anything is possible.

Hi Chaquita,
Your post was informative and educational, and I applaud the American association for taking their time education and research mental health. Mental health awareness is vital, and there is negative astigmatism regarding this issue. APNA provides resources and education to psychiatric nurses so they can provide services to mental health patients and their families (2021)
According to an online article APN “ is committed to the practice of psychiatric-mental health nursing, health, and wellness promotion through identification of mental health issues, prevention of mental health problems, and the care and treatment of persons with mental health disorders (2021)”. APN plays a significant role in mental health by researching and finding resources while educating communities on mental health awareness.

Response to Other Learner
Chaquita, you identified numerous points connecting the American Psychiatric Nurses Association’s foundation with evidence-based practice, however, I was also able to find additional information that further solidifies the connection between the two.
Part of the American Psychiatric Nurses Association’s, APNA, mission is to “improve mental health care for culturally diverse individuals, families, groups, and communities” (American Psychiatric Nurses Association, 2022a). Some of APNA’s core values are also noteworthy, such as inclusivity, innovation, and empowerment (American Psychiatric Nurses Association, 2022b). This information recognizes health equity and continuous healthcare improvement as some of APNA’s goals. This further connects APNA with evidence-based practice, EBP, due to EBP research study results.
Melnyk et al. (2010) concluded that “evidence-based practice (EBP) leads to higher quality care, improved patient outcomes, reduced costs, and greater nurse satisfaction” based off results of various research studies. This means that APNA’s mission and core values are best achieved through the use of evidence-based practice, which further identifies their connection.

RE: Discussion – Week 1
Hi Matt,
I am glad to know that there is a fellow Walden classmate in my area. This is a small world indeed. If there is anything I can do for you, kindly let me know.
I do appreciate the questions you pose. I believe that having a nurse amongst the executives at MCES may help bridge any gaps in hearing about mental healthcare from front-line workers. Then again, the hospital is a smaller hospital with only 73 beds so it is possible that upper management is made readily aware of the needs of front-line staff. I am curious to know though if staff would support having an executive that has a nursing background.
In regards to meeting the mental health needs in our area, I am sure that the changes at Norristown state hospital have significantly affected the community. In regards to non-behavioral health needs, I know that Einstein Medical Center Montgomery Hospital and Suburban Community Hospital meet health needs in the area. However, these hospitals do not have mental health units. Thus it is quite possible that mental health needs are not being met in our area as they were before the shutdown of Norristown State Hospital; which is connected to MCES. Nevertheless, there are mental health services through Central Behavioral Hospital, Eagleville Hospital, and many other private organizations nearby that care for adults and youth that suffer from mental health ailments.
I hope my answer addresses your concern.

I didn’t know that MCES was connected to Norristown, thats good to know. My familiarity with the area stems from my work in DelCo actually. Before I became a nurse I worked as a case manager for Crozer so I spent a lot of time in the Chester/Upland area. Glad to hear from you as now you’re the most “local” person I’ve met at Walden so far.

Many times one might indicate key guidelines for evidence, but then there is no rationale to provide insight for the reader. As graduate prepared nurses it is imperative that we “follow the evidence”, and without rationale that cannot be provided. Good job!

RE: Discussion – Week 1
I chose to review the American Society for Nutrition (ASN) and to find out how they implement evidence-based practices in their research and articles. As evidence-based practice is the gold standard for nursing care, I was pleased to find that the ASN includes that specifically in their Vision, “A Healthier World Through Evidence-Based Nutrition” (American Society for Nutrition 2020). The association’s entire website is littered with references to evidence-based care, research, and best practices.
I feel that ASN is grounded in evidence-based practice as they produce articles on a weekly basis that follows the steps of nursing science to help identify and solve problems in healthcare (Walden University 2018). The ASN also has multiple publications that range from almost 100 years old to newer acquisitions that are chaired by leaders in their fields. Being focused on evidence-based practice and being able to fully provide the best care needs to not only focus on the science and data, but also on patient preferences and values (Melynk et al 2010). By reading some of the recent articles posted, their research and how patient preferences are included are evident. An article posted regarding lowering cholesterol shows the patient preference component indicates that there is a large segment of the population that are either unwilling or unable to take lipid-lowering agents and how different snacks can be used to achieve the best outcomes (Kopecky et all 2022).
Researching the ASN did change my perspective on the association. They are much more robust than I originally thought. The ASN includes 4 different physical publications – The Journal of Nutrition, The American Journal of Clinical Nutrition, Advances in Nutrition, and Current Developments in Nutrition. The ASN also has meetings around the world with topics ranging from experimental biology, diabetes, and food improved by research and technology (American Society for Nutrition (b) 2021). Updates are frequent on their site with seven articles posted this month that are all focused on how research can lead to the best clinical outcomes. Overall I’m much more impressed with the ASN than I thought before looking into them for this assignment.

If the information provided is based on solid research, then the content noted is indeed solid. Knowing how to evaluate research is a key part of nursing practice. Many of our patients or clients listen to “Dr. Google”, and they are not adept at determining which is information is based on solid evidence, what is accurate information, and what is specific to their issue. I cannot tell you how many times (and it happened just this week) someone will say to me that they saw on the internet that some treatment is appropriate for something. When you look at the “research” there is no solid foundation- much like a “source close to [famous person] states that…”. Thus, it is imperative that we are aware of how to process appropriate and accurate information from the internet and to teach those in our care how to do the same sort of thing as well.
Thanks much for your insights.

Hello Matthew, your review on the American Society for Nutrition (ASN) website was very informative. One area immediately noticeable when logging onto their website is their mission “Excellence in Nutrition Research and Practice” (ASN, 2022). Nutrition plays an important role in the health of a country and has been a focus of the U.S Department of Health and Human Services (HHS) Health People 2030 Objectives addressed as part of Overweight and Obesity (HHS, n.d.). In both the ASN and HHS office of Disease Prevention and Health promotion, evidence-based education can be obtained for education, research and for developing new programs.
For example, in the Latest Articles, ASN reports on the link between Nutrition and Heart Disease and uses the World Health Organization (WHO) as a reference to support cardiovascular diseases can be prevented by addressing behavior risk factors such as an unhealthy diet and obesity (Garber, 2022). Healthcare organizations depend on resources provided by ASN and validated by government resources such as HHS and WHO to make sure they are meeting the standards for treating patients and for developing programs.
My facility, the Veterans Administration offers a program called Whole Health (Veterans Administration n.d.). Whole Health is modeled after programs with goals to keep the patient healthy according to what is identified as being healthy for the patient (Khodvakov et al., 2018). Once the patient’s needs are met, a partnership is developed to provide education such as material from ASN and other resources using holistic and traditional health models.

The health care organization website I reviewed was the Tampa Veterans Health Administration (Affairs, 2018). Reading through the mission, vision, and core values was surprising to not see evidence-based practice mentioned. Part of the core values is innovation through curiosity, and knowledge, but research based care is not specifically mentioned. A search of the website showed two past articles regarding evidence-based practice, but nothing recent.
This organizations future will be grounded in EBP, but it is still in the midst of a culture change. As one article stated nurse educators are a part of this culture change by leading the way and encouraging the providers to follow (Boller, 2017). It has been over the past ten years the VA has switched their practices from treating the immediate sickness to prevention with patient partnership in PACT health care (Patient Aligned Care Teams). There is also a mix in older and younger nurses leaving a discrepancy for the older nurse not being as proficient utilizing the internet and computers for implementing these practices (Melnyk, 2010). They do have a journal club for the nursing staff and EBP is part of the nurse residency program they provide for all new staff.
I was surprised that there is not more information for evidence-based care on the main website. It is something that I participate in at this facility and just assumed it would be easily found as part of our core values. I believe there is still a lot of work to do on this organization, but changing an entire culture to a new way of practicing takes time and work.

RE: Discussion – Week 1
Stacy, I enjoyed your post about the Tampa Veteran Health Administration. I am surprised that they do not mention evidence-based in their mission or values. I have a friend who is a veteran and works for the VA in Virginia, and they require him the do evidence-based project all the time. According to the (US Department of Veterans Affairs, 2021), their website states, “James A. Haley Veterans’ Hospital, we conduct research to discover knowledge, develop VA scientists and health care leaders, and create innovations that advance health care for Veterans and the nation.” I also recently interviewed the VA, and one of the questions was how I stay current with the constant changes in nursing. I told them that I do CEUs to make sure that I am up-to-date, and my organization requires me to do online mini-courses that are current with evidence-based practice. The manager said that when their employees do projects for the company, they can quickly move up to higher-ranking positions. When I was in nursing school, I learned that the VA is also big on teaching and supporting employees who continue their education. It seems that most organizations do not post on their website that the research done is evidence-based. (Melnyk & Fineout-Overholt, 2019) states that “a culture that fosters EBP promotes the spirit of inquiry and makes it visible to clinicians by embedding it in its philosophy and mission of the institution.” I never stop to think whether healthcare organizations post that their research is evidence-based; I assume that most large organizations would not post information on their website or mission that has not been researched.

RE: Discussion – Week 1
The Wound, Ostomy, and Continence Nurses Society (WOCN) is an organization that bases their practices on evidence-based research proven to help patients with the prevention and maintenance of skin breakdown. Nurses that are members of this organization are equipped with the knowledge and skills to perform the proper skin care and evaluation on patients. The vision of WOCN is ” To be the preferred community for wound, ostomy, and continence advocacy, education, and research recognized for promoting safe and effective care worldwide” (Wound, ostomy, and Continence Nurses Society, 2022).
The WOCN has it’s own journal called The Journal of Wound, Ostomy, and Continence that publishes the most up to date and evidence-based practices for the prevention and care of skin breakdown (Wound, ostomy, and Continence Nurses Society, 2022). This organization welcomes all nurses to become members of this society through wound, ostomy, and continence certification.
This organization bases their care on EBP because they have researched what is the best prevention for skin breakdown. This is care that has been proven to work in our patients. Turning a patient every two hours is hard for a nurse, but it has proven to prevent skin breakdown. The website also offers resources that can help guide a nurse to give better patient care. When taking care of a patient that is at high risk for skin breakdown, I try to do my best to reposition the patient every 2 hours and provide alternatives and resources to incontinence. Researching this website has provided me with an opportunity to become a member and further my skills.
Evidence-based practices are proven to improve the quality of care given to the patient and in return we can observe better patient outcomes (Melnyk & Fineout-Overholt, 2019, p.12). We must take into consideration that if facilities do not practice EBP, then the treatment given to their patients is outdated and can have detrimental patient outcomes (Melnyk & Fineout-Overholt, 2019, p. 13). As nurses, we are responsible to give the best care to our patients and that care can be provided through EBP (Crabtree et al., 2016).

RESPONSE TO OTHER LEARNER
Thank you for your post Claudia.
I echo your sentiment that it is crucial to use current knowledge on the latest evidence-based practices in healthcare so that treatment services to patients are not obsolete and ineffective. Upon review of the website for the Wound, Ostomy, and Continence Nurses Society (WOCN), I concur that this organization is steeped in EBP and desires to promote the use of EBP in clinical practice. WOCN’s mission, vision, and values statements all reference the use of education and research to demonstrate integrity, leadership, and knowledge using EBP. It is impressive to see that the WOCN society has become “the largest and most recognized professional nursing community that is dedicated to advancing the practice and delivery of expert healthcare to individuals with wound, ostomy, and continence care needs (WOCN Society, 2022).”
From your post, you shared that the WOCN has its own journal that is up-to-date and routinely maintained to provide current publications on wound, ostomy, and continence care practices. Crabtree et al. (2016) stress the importance of dissemination of EBP project outcomes to promote the implementation of EBP. As they put it, “the implementation and dissemination of EBP projects create opportunities for nurses to participate in the development of scholarly products and results in professional growth and development (Crabtree et al., 2016, p 174). Being entrenched in the use of EBP and promoting health professionals to do the same, the WOCN is furthering the goal of achieving the Quadruple Aim. As a healthcare professional, I feel more comfortable sharing the knowledge provided by the WOCN with patients and their families because they are vested in using the latest evidence to educate, inform, and enhance understanding about wounds, ostomies, and continence care.

RE: Discussion – Week 1
Hello Claudia.
As a Wound, Ostomy, and Continence Nurse (WOCN) Certified Wound Care Nurse (CWCN), I am a member of the WOCN organization. I have been a member for the last 4 years. The Journal of Wound, Ostomy, and Continence is a great place to find evidence-based information regarding wound treatment and prevention. There are many benefits the WOCN offers and considers their certification to be the gold standard for wound and ostomy certifications (Wound Ostomy Continence Nursing Certification Board, n.d.). WOCN markets their education to a wide range of healthcare and industry professionals. One key benefit of WOCN, according to Boyle et al (2017) is their support in offering continuing education to nurses who choose to become certified, and recognizing facilities with certified wound, ostomy and continence nurses (CWOCN) have lower hospital acquired pressure injuries (HAPI) (Boyle et al., 2017).
Some of the drawbacks of the WOCN from my own observations is the organization has not promoted growing diversity among CWOCN membership. Per literature published by the WOCN, there are approximately 8,000 wound care nurses and there is no breakdown available on the ethnic background of nurses. Given the healthcare disparities and the prevalence of amputations in people of color, the WOCN could do more to have a positive impact on providing education to underserved communities to prevent lower extremity amputations. Also, since the certification body is a small community, it is heavily marketed by the industry for research studies and product endorsements. Therefore, many of their publications have conflict of interest and must be vetted for disclosures and independence.

Claudia, Response
The WOCN Journal is my go-to whenever a complex wound is evaluated; I have access to a wound clinic nurse by instant messaging that exemplifies the premise behind interdisciplinary care and improving the experience of care (Boller, 2017). The wound care clinic nurses practice evidence-based care and are knowledgeable from Lymphedema skin breakdown to colostomy care. Even if you are not a wound care nurse, you encounter a daily wound or skin description. It is an evidence-based practice program to attend and have in your treatment repertoires, usually a couple of times due to the amount of information provided by the certification classes. I have also utilized the Journal of Wound Ostomy & Continence Nursing for investigative reading and to assist in identifying wounds. It is an excellent resource for pictures and descriptions such as hard-to-heal skin wounds. It is a tremendous evidence-based resource Muñoz, V., Pino, A., Martinez, C., Echevarria, B., Lacramioara, V., & Anitua, E. (2021) .

RE: Response 1 Discussion – Week 1
Thank you for your post Claudia
I confer, with you that it is critical to employ contemporary expertise of the most up-to-date evidence-based methods in healthcare to ensure that patient treatment services are not outdated and useless, Melnyk, & Overholt, E. (2019). I agree that the Wound, Ostomy, and Continence Nurses Society (WOCN) is deep in EBP and wants to promote its usage in medical care, based on a check of their website. Utilizing education and research to exhibit dignity, competence, and expertise via EBP is mentioned in WOCN’s purpose, goal, and principles declarations. ACCORDING TO THEIR WEBSITE, (https://www.wocn.org/) the WOCN society has grown to be “the biggest nurse practitioner organization focused on enhancing the performance and delivering excellent healthcare to patients with wound, ostomy, and continence care,” according to their website.
You mentioned in the post that the WOCN has its journal, Chien, L. Y. (2019), which is up-to-date and regularly updated to give recent articles on the wound, ostomy, and continence care methods. To enhance EBP implementation) emphasize disseminating EBP project outcomes. “The execution and distribution of EBP initiatives provide chances for nurses to contribute in the establishment of academic products while also resulting in professional growth and development,” they write, ( Melnyk & Overholt, E. (2019 ). The WOCN is helping to achieve the Quadruple Aim by promoting the usage of EBP and encouraging health professionals to do the same. As a healthcare practitioner, I am more confident in sharing the WOCN’s information. And championing for the use, implementation, and incorporation of the current techniques in the medical field in promotion and pushing for quality research, education and experience in the nursing field for the best patient outcomes and professional advancement of the medical personnel.

Response # 2
Claudia,
I enjoyed reading your post this week. I agree that evidence-based practices are proven to improve quality of care. I looked at the website for the Wound, Ostomy and Continence Nurses Society (WOCN) organization. I learned that WOCN was founded in 1968 and that is now accredits 8 WOC Nursing education programs (WOCN, 2022). I did notice you must be a member to be able to have access to a lot of the information. I am sure once you are a member one can gain a lot of evidence-based practice information from website. Evidence based practice is important because the main goal is to provide the most effective care. I do like that they have the WOCN has their own journal. The Journal of Wound, Ostomy and Continence Nursing (JQOCN) has many articles full of evidenced based practice which is great. I think another great resource for evidence-based practice in general is the Agency for Healthcare Research and Quality (AHRQ). The AHRQ is the lead federal agency that helps with improving the safety and quality of patient care (AHRQ, 2022).
